The Power Pioneers forms are available in Adobe Acrobat PDF format. This format is ideal for printing or viewing off-line. You will require the Adobe Acrobat PDF viewer, which is available for free from Adobe.

Some of these forms are also available in a format that you may be able to fill, print and mail as required.  Instructions are included where appropriate.

Membership Application

To apply for membership please go to the Membership Benefits page to apply online.

Extended Health Care Form

Print and fill out the Extended Health Care Form in accordance with the instructions on the second page and mail to Blue Cross as indicated.

BC Hydro Dental Benefit Form

Print and  fill out the BC Hydro Dental Benefit form in accordance with the instructions on the form and mail to BC Hydro as indicated.  Alternatively, fill in this BC Hydro Dental Benefit form by typing in the fields provided.  Tab from field to field. (Not all computer software may be able to open this form).  Attach your receipts and mail to BC Hydro as indicated.

Christmas Lunch Application Form

Print and fill out the Christmas Lunch Application form and return to the appropriate location as indicated on the form.  This form will be available in November of each year.

Power Pioneer Lifestyle / Volunteer Program

This calendar is designed for keeping track of your lifestyle points each month.  If you choose to use this calendar, just print and fill in the Lifestyles Calendar

The form to be submitted to your Lifestyle Representative is the Power Pioneer Lifestyle / Volunteer Claim Form.  This must be submitted by December 31 of each year.  If you prefer you can print this form in a PDF version.

Youth Community Service Award

Application for the next Power Pioneer Youth Community Service Awards can be made from January 1 to March 31 of each year.   Click here for the application form.
